While there is not a  sample of an exemption letter, below is the information that MUST be included:


 Exception letters

People who have a medical exception need to present a Valid Medical Contraindication for COVID-19 Vaccination letter instead of proof of full vaccination.

If the person lives in Nova Scotia, they got their letter from a physician or nurse practitioner.

The letter will include:

• The Nova Scotia logo with the Department of Health and Wellness identifier

• The patient’s name three times throughout the document

• The physician or nurse practitioner’s signature and the date

• The physician or nurse practitioner’s name printed along with their credentials and the date

 

If the person lives outside Nova Scotia, they got their letter from the Nova Scotia Department of Health and Wellness.

The letter will include:

• The Nova Scotia logo with the Department of Health and Wellness identifier

• The patient’s name three times throughout the document

• The physician or nurse practitioner’s name printed along with their credentials and the date
